What is Physical Therapy?

Physical therapy is a medical treatment provided by physical therapists. Physical Therapists are expert in the musculoskeletal and neuromuscular system. This training allows them to treat and work with a variety of diagnoses.


What should I wear to my appointment?


Please wear clothes that will allow full access to the body part you are seeking services for. If necessary, Holmes PT can provide a gown to ensure modesty when accessing the body part.

What do I need to bring?

Be prepared to complete medical history forms including your medication list. If you prefer to complete the forms prior to your appointment, follow the link below. Please also bring any prescription, referral, or imaging report that was provided by a physician.


How do I make an appointment?


Please contact us either through a phone call (978-499-9772) or through email at contact@livefreept.com   Our goal is to schedule your evaluation with 24-48 hours of contacting us depending on your schedule and availability.

We have a $25 no show fee or cancellations with less than 24-hour notice. This is to ensure access to services for all patients.



How many visits will I need?

The number of treatments will vary depending on the problem and individual. After a thorough assessment, the physical therapist will discuss a treatment plan for the rehabilitation process.


Do I have to be referred by a doctor?


No, Massachusetts is a direct-access states that allows a physical therapist to evaluate you without a referral. Each insurance has different requirements for a referral or prescription. It is the responsibility of the patient to understand and determine their benefits. Holmes front office staff will assist in determining your benefits and referral requirements.
